This webpage hosts a link to a Canadian Government mandate letter that looks at the social determinants of health in Canada and the key areas for change.  It discusses embedding health in all government policy to help increase health equity.

This Annual Service Plan Report published by the Minister of Health looks at the actual results of the Provinces service plan.  I found it gave me an idea of the province's strategic direction and plan and where we are currently on each issue.
